Requirements to Change Your Name
Petitioner must be 19 years of age or older and must be a resident of Tuscaloosa County to file a petition for change of name.
Documents Required
- U.S. State-Issued Driver’s License/Non-Driver ID or U.S. Passport*
- Birth Certificate
- Marriage Certificate/Divorce Decree if changing due to a marriage or divorce
- $32.00
- Additional documents may be required for some cases
You must fill out the Request to Change Name form completely. Upon request, the Probate Court will notarize your signature. Please do not sign the Request before filing with the court.
An Alabama Law Enforcement Agency (ALEA) and an FBI background check will be required on all name changes except marriage/divorce changes or minor corrections on birth certificates or other Alabama government IDs. Our office will provide you with instructions on how to obtain the background check.
*PLEASE NOTE: If your U.S. State Driver’s License/Non-Driver ID does not have your correct physical address, you must bring a utility bill with your name and correct address.
